CareSparc Community Connections (CCC) offers an innovative, evidence-based community engagement model grounded in the CDC’s Social Determinants of Health and the Robert Wood Johnson Foundation’s Culture of Health Action Framework.
By integrating acute care, wellness services, and lifestyle activities, CCC addresses the full spectrum of wellbeing and fosters meaningful, trust-based relationships between providers and resource-limited populations.
Through cross-sector collaboration, CCC builds resilient networks that respond to public health challenges, promote long-term wellbeing, and create lasting opportunities for communities of color to thrive.
